In the UO storyline, Mondain Shattered the Gem of Immortality, and thus created the Shards, each server is considered a Shard of the gem, in which the fates of its populace are unique.
A world is the largest geographical category, also known in UO as Shards of the gem of immortality.
World contains continents and facets.

Travel between worlds
The only known means of travelling between worlds is through the use of a rare black portal, such as the one used by Lord British at the day of his leaving the world of UO. Or the Paid character Transfer Service.
